Instructions: These four on-line forms are to be completed on your computer.  After filling out the form, print it then sign it.  Make the required number of copies of the signed form to mail to the proper Department Officer and a copy for yourself as usual. The Advantage is that all copies are legible. Unfortunately Adobe Reader does not permit saving the completed form. It will only save the blank form. These forms may not be e-mailed in-lieu of mailing them.

If you have Adobe Professional then you will be able to save the completed form, however it is not a free version.

Use this form to notify the Dept. about changes in your Detachment/Unit Officers status.
Note: Data Fields that require large amounts of data such as addresses and the Phone/Email fields, have more than one line of data input available. Simply press the "Enter" key to get the next line.

Forward three signed copies to the Dept. Adjutant.

Department Convention Delegate Registration Form

Use this form to complete the detachment delegate registration. Print and mail this form with the appropriate delegate registration fee to the Dept. Paymaster.

Follow submission instructions on the form. Both detachments must sign before submitting to Dept.
